Pull pci fixes from Bjorn Helgaas:

 - Initialize msi_addr_mask for OF-created PCI devices to fix sparc and
   powerpc probe regressions (Nilay Shroff)

 - Orphan the Altera PCIe controller driver (Dave Hansen)
