Matthew Wilcox 6eb0d698ef NVMe: Replace DEFINE_PCI_DEVICE_TABLE ...

Checkpatch has started warning against using DEFINE_PCI_DEVICE_TABLE,
so replace it.  Also update the copyright date and bump the module
version number to 0.9.
